Assisted Living & Adult Home

We aim to make our services accessible to all those who need them and qualify. Crestview offers an all-inclusive payment structure, so that families are not “nickle and dimed” for each service required.

For qualifying adults, we are proud to be able to access government funding, including Medicaid funding for personal care services and Supplemental Security Income (SSI) Level 3 Congregate Care funding for Room & Board. Qualifying adults also receive “Personal Spending Allowances” from government funding to financially cover their basic needs (for items not included).

 

Adult Home Program

The Adult Home program provides Room & Board plus personal care and 24 hour support services at Crestview Manor. The Adult Home Program is available for adults who do not qualify for the ALP. The Adult Home is a privately funded program.

Adult Home residents enjoy full residential services, meal services, medication management services and personal care services, as well as case management, life enrichment activities, housekeeping and laundry.  Crestview Manor has 27 licensed Adult Home (non-ALP) beds.

Assisted Living Program

The Assisted Living Program (ALP) is the only true Medicaid funded assisted living option in New York State. The ALP enables qualifying adults access to 24 hour, around the clock personal care services for Activities of Daily Living. Personal Care Services are provided by certified Personal Care Aides and Home Health Aides of a Licensed Home Care Service Agency (Golden Care LHCSA), which maintains an office at Crestview. The program is fully paid for by Medicaid for Medicaid eligible adults.

Payment for Room & Board is often covered by government funds for Medicaid adults, including SSI, SSA and SSP. Additionally, residents who receive SSI/SSA/SSP are entitled to receive a monthly Personal Needs Allowance (PNA) each month.

Crestview Manor has 49 licensed ALP beds.
